What are you hoping to achieve through your Fit Club membership?
Why are these goals important to you?
I’m looking for an answer deeper than just losing weight or fitting into that old pair of jeans. I want to know WHY you want this and why is now the time for you? (ex: I want to lose weight so that I can set an example for my kids, I want to have more energy so I can start the side business of my dreams)
What are your biggest struggles when it comes to your eating habits?
(ex: emotional eating, addicted to sugar, no time to cook healthy, staying consistent)
What are your biggest struggles when it comes to working out?
(ex: low energy, no time, no equipment, injury)
What roadblocks in the past have stopped you from achieving your health goals and why?
(ex: wasn’t seeing results and quit, work got too busy, family responsibilities)

(1 meaning you never workout and 10 meaning you workout 5+ days per week)
